I would make sure you get the most out of your practice. When you are on the driving range make sure you hit every shot like it was out on the course, even do your pre-shot routine if you have one. Most people get out on the range and hit ball after ball without any purpose. Practice with a purpose, visualize and practice shots you will use on the course, it will help you.
When you are on the putting green you should try to hole 100 putts in a row from 3 feet out, if you miss one start over, go until you make 100 straight. You should try to do that alteast once a week. It can be painful but all of the repetitions will help your stroke and the fact you have to make 100 straight or you start over will help you putt better under pressure and give you a lot of confidence on short putts.
